When you send the contact form, Well Story stores your name, phone number, city, care choice, and timeframe so our team can review your interest. The waitlist stores your email and optional city so we can share launch news. Your details are not visible to other visitors. Sending them does not create a booking.
Care and city choices
City and care choices appear in page addresses so you can open relevant care information. Do not place personal or medical information in those fields. A copied page address may include the city or care choice.
